  This bike hasn’t been touched in 20 years, and was probably never serviced. I think a new piston and rings are in order, the rust on the rod seems a bit concerning. Any advice would be helpful. This is from a 1970 Yamaha ht1 90cc. I’m pretty much learning as I go, YouTube and this website have been a lifesaver!

Is rod actually rusty?  No too common to see that.  Might just be copper cladding used to heat treat properly?

I agree too. It's a treated surface so quite resistant to corrosion.
The ignition side web on the crank is dry looking and surface corrosion on that but that's quite normal on motors that have been stood.
The HT1 is very sensitive to compression loss, so be sure to check the bore. Run your fingernail over any tram lines that are probably present and if you can feel them then re bore it.
You will probably find the rings have gummed up and are stuck back in the lands hence the marks.
I put an MX 100 rod in mine as the lubrication cut outs are much better and it's lighter.
